We have an open patient list for patients that are new to the area, our boundry includes Cults, Bieldside, Milltimber, Mannofield and some surrounding areas, if you are unsure whether you are eligible to join please call 0345 3371140 to check. Unfortunately due to the number of practices in Aberdeen with closed lists we have had to temporarily restrict movement to our practice from other practices, so if you currently have a GP and you have not moved address you need to remain with your current surgery.
To submit a registration request. You must complete TWO forms.
Please read carefully and follow instructions 1 to 4.
1). Complete and submit New Patient Questionnaire Form.
2). Complete and submit New Patient Registration Form.
3). You must attend the practice within 7 days of submitting your registration request forms to sign the application and show photo ID and proof of address. If you do not attend within 7 days your submission forms will be destroyed and you will need to complete the forms again.
4). Proof of Immunisations must be provide for children under 5. Please bring this to Reception.
There is no restriction to patients joining the practice as long as they live within the practice boundaries shown on our map in reception and would qualify for free NHS treatment. We do not discriminate on the grounds of race, gender, social class, age, religion, sexual orientation or appearance.
